February Half Term Activity Registration Form
Have A Go Boxing - Thursday 17th February 2022 11.45am - 2.30pm

We are delighted to offer children eligible for free school meals a FREE hour and half Have A Go boxing session in partnership with St Vincents Amateur Boxing Club and Sheffield Healthy Holidays.
Children will receive a drink and fruit snack at the start of the session. Followed by a hot meal or sandwich and drink in our cafe. Free cup of tea or coffee for parents and carers.
Please complete the booking form fully, accurately and return to community@hgdt.org before February 14th to book your child's place on the above activity. Places are extremely limited and will be allocated on a first come first served basis so we advise sending your form as soon as possible to avoid disappointment. You will receive an email confirmation if your booking has been successful. Please do not attend unless you have a confirmed booking. If you haven’t heard anything within 48 hours of submitting your form please get back in touch. Note : There are 4 pages to complete.
Parent(s), guardian(s) or carer(s) MUST remain with their child throughout the duration of this activity. No drop off or collection is available on this occasion.
